***If your spouse, partner or parent is the primary insurance member, then a signature is required from them prior to direct billing. Please contact the office prior to your appointment so that forms can be emailed to you.

****Please check your email  ( spam box too) and text messages for reminders. It is now required that you confirm your appointment when you receive a reminder message. 

****24 hours notice is required for all cancellations . Failure to provide notice, regardless of reason ( including illness), will result in the FULL fee charged to your account. Non payment of missed appointment fee will result in discharge from Massage Therapy services at this clinic. ****

 

****New patients may only book the initial appointment. Subsequent appointments may be booked once you have had your initial Massage Treatment.
